Dear Prospective CCNS Parent,

 It is with joy that I welcome you to Calvary Christian Nursery School as one of CCNS’s parents. We rejoice that you have chosen CCNS and we hope that the experience that your child has at Calvary is a loving and learning experience.

 In order to register your child, you will need to download two sets of materials. Both sets of materials are in PDF format and you will need Adobe Reader to download these materials. If you do not have Adobe Reader, you may download it by going to http://www.adobe-reader.org/.

 First is the CCNS "Registration Pack". This five-page packet of forms contains the Admission Application, Emergency Information Form, Personal History Form, Tuition Payment Form, and an important notice concerning Influenza Vaccine. You may download this packet by left-clicking the link below:

Second, the “Universal Child Health Record” form is also required. This health form must reflect a physical with the last year (June 1, 2008 and July 31, 2009) and it requires a physician’s signature. In addition,  immunization record from your child's doctor must also accompany this Health Form. You may download this form by left-clicking the link below:

All of these forms must be returned to the church office by July 31, 2009 (with the exception of the Tuition payment form), with check for $75 non-refundable deposit and one (1) month’s tuition. The check is to be made payable to “Calvary Lutheran Church”. Please mail the check and forms to:

 If you wish to hand deliver the forms, the office secretary, Sue Poff, will be available in July and August, Mondays and Wednesdays, 9 AM to 3 PM.

 AN IMPORTANT REMINDER:  All children entering the school must be completely toilet-trained. Pull-ups or diapers are not permitted.
